摘要:
PURPOSE: To evaluate S1AI-S4AI screw channels with three-dimensional digital technology simulation analysis and to study the feasibility and applicable features of sacral alar-iliac screw fixation in adults. MATERIALS AND METHODS: Forty (20 men and 20 women) normal adult's pelvic CT scan data sets were selected to reconstruct the three-dimensional pelvic model. The ideal S1AI-S4AI screw channels were simulated, followed by precise measurement of their parameters. RESULTS: The results showed that there were no significant differences in the transverse angles, sagittal angles, radiuses of the maximal inscribed circles, or lengths of the screw channels in S1AI-S2AI screws between genders (P > 0.05). In contrast, the radiuses of the maximal inscribed circles on the left and right, respectively, were 5.93 +/- 1.02 mm and 5.92 +/- 1.04 mm in males and 4.64 +/- 0.98 mm and 4.59 +/- 0.95 mm in females, and there was a significant difference in S3AI screws between genders (P < 0.05). With a radius of 2.50 mm considered to be standard, there were 25 cases (62.5%) with an S4AI screw channel radius </= 2.50 mm in 40 adults, and 15 cases (37.5%; 9 males and 6 females) with a radius > 2.50 mm. Furthermore, the transverse angles, the sagittal angles, the lengths of the screw channels, and the radiuses of the maximal inscribed circles were significantly different between genders in 15 cases (P < 0.05). CONCLUSION: Only one maximum ideal screw can be placed on one side at a time. With a radius of 2.50 mm considered to be standard, it is feasible to place S1AI-S3AI screws with a radius > 2.50 mm in the entire adult population and S4AI screws with a radius > 2.50 mm in some of the adult population. Furthermore, preoperative three-dimensional reconstruction and three-matic research software can effectively simulate the sacral alar-iliac screw channels, and they can provide accurate data for clinical applications.

摘要:
Cardiovascular diseases (CVDs) are the leading cause of death worldwide. Previous studies have shown that inflammatory chemokines are involved in the physiological functions of cytoskeletal reorganization, cell migration, adhesion and immune responses. However, in the past decade, there have been studies showing that inflammatory chemokines play a key role in CVD. Importantly, CXC motif chemokine ligand 2 (CXCL2) has been shown to be involved in the pathogenesis of cardiovascular disease. CXCL2 exerts its effects on the cardiovascular system by mediating inflammatory responses, but the specific signaling pathways by which CXCL2 exerts its effects in CVD remain unknown and need to be further investigated. This review aims to investigate the expression changes of CXCL2 in acute myocardial infarction (AMI), atherosclerosis (AS), obesity, diabetes and ischemic stroke (IS) and its potential key role in cardiovascular disease in order to provide new ideas for the prevention and treatment of cardiovascular diseases. © 2019, © 2019 Taylor & Francis.

摘要:
真核翻译起始因子2α(e IF2α)是真核翻译起始因子2的一种调节亚基,是催化蛋白质合成起始的关键蛋白。以往研究证实,磷酸化的e IF2α对蛋白质的合成能够起到负反馈调控的作用;最近研究发现,磷酸化的e IF2α还能特异性激活某些mRNA的翻译以合成特定蛋白质来调节靶基因的表达,进而促进疾病的发生发展。研究发现,e IF2α在动脉粥样硬化、肺动脉高压、肿瘤等增殖性疾病中差异表达,这些证据提示e IF2α可能在人类心血管病和肿瘤等增殖性疾病中发挥重要的作用。因此,进一步探讨e IF2α在增殖性疾病中的作用及机制,对于人类心血管病和肿瘤等增殖性疾病的防治具有重要意义。

摘要:
Purpose: Eukaryotic initiation factor 2α (eIF2α) plays important roles in the proliferation and survival of pulmonary artery smooth muscle cells (PASMCs) in animal hypoxia-induced pulmonary hypertension models. However, the underlying mechanism remains unknown at large. Autophagy has been reported to play a key role in the vascular remodeling in pulmonary arterial hypertension (PAH). The purposes of this study are to determine the functions of eIF2α and autophagy in the vascular remodeling of the monocrotaline-induced PAH rats and to clarify the correlation between eIF2α and autophagy. Methods: We established a rat model of monocrotaline-induced PAH, and we established a cell model of platelet derived growth factor (PDGF)-induced PASMCs proliferation. The vascular morphology and the expression of eIF2α, LC3B, and p62 were assessed in the pulmonary arterial tissue of Sprague-Dawleyrats and PDGF-induced PASMCs. Results: Autophagy was significantly active in monocrotaline model group (MCT)-induced PAH rats, which obviously promotes vascular remodeling in MCT-induced PAH rats. Furthermore, the proliferation of PASMCs was induced by PDGF in vitro. The expression of LC3B, eIF2α was increased in the PDGF-induced PASMCs proliferation, and the expression of p62 was reduced in the PDGF-induced PASMCs proliferation. Moreover, eIF2α siRNA downregulated the expression of eIF2α and LC3B, and upregulated the expression of p62 in PDGF-induced PASMCs proliferation. eIF2α siRNA inhibited the PDGF-induced PASMCs proliferation. Finally, chloroquine can upregulate the protein expression of LC3B and p62, it also can inhibit proliferation in PDGF-induced PASMCs. Conclusion: Based on these observations, we conclude that eIF2α promotes the proliferation of PASMCs and vascular remodeling in monocrotaline-induced PAH rats through accelerating autophagy pathway. © 2019 Guo et al.

摘要:
如何通过腰骶固定融合手术实现稳定的骨性融合一直是脊柱外科面临的难题。各种融合至骶骨的骨盆固定技术已广泛应用,它们在稳固腰骶部,提高融合率,纠正脊柱畸形、骨盆倾斜及减少假性关节和并发症发生等方面起到至关重要的作用。随着腰骶椎疾病发病率逐年上升和患者对生活质量要求的不断提高,传统的骶骨骨盆固定技术如Galveston棒固定、髂骨螺钉固定及S_1或/和S_2螺钉内固定已不能满足临床科学技术发展的需要。2007年Sponseller提出了经第2骶椎骶髂(second sacral ala-iliac,S_2AI)螺钉固定新技术,此后报道了第1骶椎骶髂(first sacral ala-iliac,S_1AI)螺钉固定。由于此两种螺钉固定术具有良好的生物力学性能、无需交叉连接器、易于经皮或微创手术放置、并发症较少以及不妨碍髂骨翼植骨取材等优点,已越来越多地成为取代骶骨骨盆固定术的优先选择。然而,由于S_1AI、S_2AI钉道较狭长,骶骨骨质薄弱,腰骶部需承受较大的载荷以及腰骶角引起的局部剪切应力等因素,以致螺钉置入相对困难,且易发生固定融合失败;加之术后假关节形成,内置物松动或断裂等并发症时有发生,致使S_1AI、S_2AI螺钉置钉尚难达到理想的状态。因此,许多学者进行了大量相关的基础和临床应用研究。现综述如下。

文章就护理专业基础与临床全方位相结合的系统解剖学教学模式进行论述,围绕护理专业系统解剖学教学为何要将基础与临床全方位相结合以及如何做到这一点作了详细探讨。通过精选教材内容、PBL教学模式、体表标志的触摸、实验教学学生自主观察并课后绘图以及课后反馈等方式可以使系统解剖学与临床护理全方位地结合在一起。相比传统护理专业系统解剖学教学模式,新的教学模式教学重点突出、学生参与积极性高涨、学生掌握知识结构更完整,对于护理专业学生学好系统解剖学并提高其自学能力、实践能力和创新能力具有重要作用。

摘要:
目的 利用CT数据观察腰神经根及其与主要毗邻结构的位置关系,并对其进行三维构建与测量,为经皮穿刺腰椎间盘术提供解剖学数据.方法 选取30例健康成人腰骶部的CT连续断层扫描数据集导入Mimics 17.0软件,分割出相关结构,获得各结构的图层,对各结构进行三维重建和虚拟显示,并对腰神经根及周围主要结构进行观察及相关测量.结果 L3~L4椎间盘层面到L5~S1椎间盘层面腰神经根与上关节突的最近距离逐渐增大;三维模型清楚地显示了腰神经根及其主要毗邻结构在腰骶部的形态和空间位置关系;腰椎间盘的厚度越往下越厚,而腰椎间盘后缘中点到上位腰椎棘突末端的距离,先增大再减少,在L3~L4椎间盘到上位腰椎棘突末端距离最大.结论 通过对腰神经根与上关节突的关系、腰椎间盘厚度及腰椎间盘到上位棘突末端距离变化的分析,为临床经皮穿刺腰椎间盘手术提供数据参考.

Background: Percutaneous cervical pedicle screw fixation has been proven to be an effective method of cervical screw instrumentation, which has the advantages of less invasiveness and low blood loss. Emerging evidence has indicated that the cervical spinous process plays an important role in percutaneous spine surgery. However, there is a limited amount of information on the fundamental research of pedicle and its associated imaging parameter measurement. The purpose of this study was to measure the anatomic data of the pedicle screw channel (PSC) using reverse engineering and three-dimensional reconstruction, and also to discuss the three-dimensional relationship between the cervical spinous process and the pedicle screw channel. Methods: Twenty adult subjects (10 males, 10 females, age range 19–46 years) were studied using the method of three-dimensional CT reconstruction and reverse engineering. The centrum was divided into 10 equal parts from front to back. The bisectors were defined as borderline depths of the centrum, from front to back, 100%, 90%, 80%, 70%, 60%, 50%, 40%, 30%, 20%, 10%, 0% of borderline depths were presented. Then, a 3D coordinate system was constructed to measure all the data, including the radius of the inscribed circle, the length of the PSC, the insertion angle, the distances from entry point to cervical spinous process and skin depth. All the indexes were measured from 70% to 90% borderline depth. Results: The radius of the inscribed circles from C3 to C7 at 90% borderline depth were 2.94 ± 0.55 mm, 3.04 ± 0.40 mm, 3.15 ± 0.36 mm, 3.28 ± 0.47 mm, 3.89 ± 0.54 mm, respectively. The lengths of the PSC were between 25 and 32 mm. The insertion angles for 70% to 90% borderline depth were 28.33°, 34.28°, 37.92°, respectively. The relationship between the PSC and spinous process was measured as the distance from the entry point to the end of the spinous process, which were, respectively, 26.91 mm, 28.18 mm, 30.03 mm, 35.67 mm, 41.99 mm from C3 to C7.The distance from the skin to the entry point of C3-7 increased gradually. Conclusions: The measurements of this study could provide detailed information for percutaneous cervical screw fixation. The data of the relationship between the cervical spinous process and the pedicle screw channel present valuable technical information for the design, optimization and clinical application of the aiming device for percutaneous cervical pedicle screw fixation. 摘要:
目的探讨腰椎棘突中轴线与椎弓根进钉通道的三维位置关系,为腰椎内固定和辅助固定技术及临床应用提供个性化、精准化的解剖学数据参考。方法将CT数据通过三维重建和逆向工程软件处理,测量椎体70%和90%边界深度的椎弓根进钉通道所在轴线L和棘突中轴线M与三维参考平面夹角,以及交点与线及三维参考平面距离。结果(1)腰椎棘突中轴线与矢状面、水平面、冠状面夹角分别为2o ~3o、11o ~18o、71o ~77o;椎体70%和90%边界深度的椎弓根进钉通道与矢状面夹角分别为7o ~ 22o和16o ~33o,与水平面夹角分别为1.8o ~2.9o和1.4o ~2.4o,与冠状面夹角分别为67o ~82o和56o ~73o;(2)随腰椎水平降低,点到线的距离呈规律性变化;(3)随腰椎水平降低,点到三个平面的距离也呈规律性变化。结论通过对腰椎棘突中轴线与70%和90%边界深度的椎弓根进钉通道的三维关系分析,为临床经皮椎弓根螺钉置入手术和相关器械设计提供数据参考。

目的探讨颈椎椎弓根进钉参数与棘突的三维关系,为经皮椎弓根螺钉内固定及器械的设计提供参考。方法20例健康成人颈椎CT重建模型导入UG imageware 13.2,以70%、80%、90%边界深度为研究基础,测量椎弓根通道进钉螺钉半径、外偏角、通道长度(L)、通道与矢状面、水平面的角度;进钉轴与冠状面的交点与椎弓根的角度。B、C点到参考平面、棘突的距离。结果(1)C_(3~7)椎弓根通道内切圆半径在2.79~3.89 mm之间。(2)通道长度、外偏角随着边界深度的增加而增大。(3)C_(3~7)的Ac最大值出现在70%边界深度。(4)B点到正中矢状面、冠状面的距离都随着边界深度增加而增大。(5)B点、C点棘突末端的距离随着边界深度增加而增大。结论颈椎椎弓根螺钉进钉参数与棘突的三维关系,为经皮微创椎弓根螺钉固定器械设计提供依据。

摘要:
人体解剖学教学是卓越医师培养的重要内容,为探索和实践南华大学医学院"卓越医生教育培养计划"拟定的人才培养新模式,改革人体解剖学课程的教学现状,从优化课程、更新教学理念、改进教学方法与手段、加强实验室建设,应用综合测评体系及配置合适的师资力量等方面进行改革,在理论课中更新教学理念,实验课中增加基本技术操作设计性实验,改革人体解剖学考核体系等,增强了卓越医学生的自主学习、团结协作和科研创新能力,教学效果明显提高。

摘要:
新形势下的医学人才不仅要有扎实的医学理论知识,还必须具备创新意识、科研能力,以适应当代社会对医学人才的需要。加强医学生的自学能力、实践能力和创新精神的培养,提高医学生的综合素质,已成为目前高等教育改革的主要目标。 在人体解剖学教学中,实验教学是人体解剖学教学的重要环节,它在培养医学生科学思维,创新精神和实践能力等综合性专业素质中具有十分重要的作用。积极进行人体解剖学实验教学改革,开展研究型实验教学模式,对于培养医学生的科学精神和创新性思维,提高发现问题、分析问题和解决问题的能力均具有非常重要的意义,更是高等医学教育面临的一个重大课题。

酶消化法分离培养原代肺动脉平滑肌细胞及免疫组化鉴定

摘要:
目的:建立体外胶原酶消化法分离培养大鼠肺动脉平滑肌细胞( PASMCs)及免疫鉴定的方法。方法采用I型胶原酶对PASMCs进行体外培养。在无菌环境下,分离提取雄性Sprague Dawley( SD)大鼠肺动脉主干,剥离外膜、剔除内皮细胞,经酶消化法,体外培养PASMCs。倒置相差显微镜观察PASMCs生长状态及特点;台盼兰测定细胞活力;免疫细胞染色法进行平滑肌α-肌动蛋白(α-SM actin)鉴定。结果酶消化法分离培养的PASMCs 3 d后,可见细胞贴壁呈梭形生长,6 d后生长迅速呈典型的峰-谷状生长,9 d后达90‰融合。通过形态学观察及免疫荧光染色法鉴定表明:培养的细胞为PASMCs;细胞存活率为98.5‰;原代培养8~10 d后即可传代,3-10代细胞生长迅速、细胞形态不易发生改变,可用于进行细胞实验。结论采用I型胶原酶消化法简单易行、可靠、PASMCs生长迅速、传代周期短的特点;尤为重要的是,培养的原代PASMCs可用于肺动脉高压和肺血管重构的研究工作。

交叉学科(又称跨学科)是培养创新人才的摇篮,探索和发展交叉学科是高校综合性发展的必然趋势。实践证明,重大的社会发现或创新成果出现在交叉学科中。然而,研究生教育担负着为国家培养高水平、创造型和复合型人才的重任。因此,构建研究生交叉学科培养体系,培养创新人才,已成为高等院校和学术研究关注的问题。本文结合我国高校当前人才培养的实际情况,构建医工交叉学科培养体系培养创新人才。

摘要:
Numerous studies have reported the presence of oxidized LDL (ox-LDL) and expression of its lectin-like receptor, LOX-1, have been shown in atherosclerotic regions. The present study aims to investigate the effects of ox-LDL on expression of desmoglein 1 (DSG1) and desmocollin 2 (DSC2) in endothelial cells, and to explore the role of LOX-1 mediated signal in the permeability injury associated with DSG1 and DSC2 disruption induced by oxidized lipoprotein. RT-PCR and Western blotting were applied to determine the mRNA and protein expression levels of DSG1 and DSC2 in human umbilical vein endothelial cells (HUVECs) respectively. Immunoreactivities of DSG1 and DSC2 were detected by laser scanning confocal microscope (LSCM). HUVEC monolayers permeability was evaluated by FITC-labeled LDL in transwell assay system. The possible signal was assessed using in vitro blocking LOX-1 or Ca2+ channel or PKC. The DSG1 and DSC2 expression were decreased by ox-LDL in concentration- and time-dependent manner. The effects of ox-LDL were mediated by its endothelial receptor, LOX-1. In parallel experiments, ox-LDL increased the influx of extracellular calcium, activation of protein kinase C (PKC) and permeability to LDL, which was inhibited by the LOX-1blocking antibody (10 μg/ml), Ca2+ channel blocker (Diltiazem, 50 μmol/L) and PKC-β inhibitor (hispidin, 4 μmol/L). These results suggested that ox-LDL-induced decrease in DSG1 and DSC2 expression and monolayer barrier injury via calcium uptake and PKC-β activation following up-regulation of LOX-1 is one of the mechanisms of inducing greater permeability in HUVECs. © 2015 Elsevier Inc. 针对国家的需求,同时也是高等医学院校提高自身办学质量和国内办学地位的需求,各高等院校结合自身办学资源和特色,提出相应的卓越医师培养目标及实施模式,为培养具有行业适应能力的医生提供重要平台。基于卓越医师教育背景下,人体解剖学的教学应该适应时代发展。通过对人体解剖学课程卓越医师的培养方式、实施模式、保障措施等进行阐述,对进一步落实卓越医师培养计划具有重要意义。

习题对提高留学生人体解剖学学习效果的评价

摘要:
为了评价习题对提高留学生人体解剖学学习效果的作用,对南华大学2010级和2011级留学生医学班完成的题量、期末考试通过率与卷面成绩及学习兴趣进行了问卷调查和统计分析,结果发现,课后习题能显著提高留学生人体解剖学的学习效果。
