Background Mediastinal neoplasms are typical thoracic diseases with increasing incidence in the general global population and can lead to poor prognosis. In clinical practice, the mediastinum's complex anatomic structures and intertype confusion among different mediastinal neoplasm pathologies severely hinder accurate diagnosis. To solve these difficulties, we organised a multicentre national collaboration on the basis of privacy-secured federated learning and developed CAIMEN, an efficient chest CT-based artificial intelligence (AI) mediastinal neoplasm diagnosis system. Methods In this multicentre cohort study, 7825 mediastinal neoplasm cases and 796 normal controls were collected from 24 centres in China to develop CAIMEN. We further enhanced CAIMEN with several novel algorithms in a multiview, knowledge-transferred, multilevel decision-making pattern. CAIMEN was tested by internal (929 cases at 15 centres), external (1216 cases at five centres and a real-world cohort of 11 162 cases), and human-AI (60 positive cases from four centres and radiologists from 15 institutions) test sets to evaluate its detection, segmentation, and classification performance. Findings In the external test experiments, the area under the receiver operating characteristic curve for detecting mediastinal neoplasms of CAIMEN was 0 & BULL;973 (95% CI 0 & BULL;969-0 & BULL;977). In the real-world cohort, CAIMEN detected 13 false-negative cases confirmed by radiologists. The dice score for segmenting mediastinal neoplasms of CAIMEN was 0 & BULL;765 (0 & BULL;738-0 & BULL;792). The mediastinal neoplasm classification top-1 and top-3 accuracy of CAIMEN were 0 & BULL;523 (0 & BULL;497-0 & BULL;554) and 0 & BULL;799 (0 & BULL;778-0 & BULL;822), respectively. In the human-AI test experiments, CAIMEN outperformed clinicians with top-1 and top-3 accuracy of 0 & BULL;500 (0 & BULL;383-0 & BULL;633) and 0 & BULL;800 (0 & BULL;700-0 & BULL;900), respectively. Meanwhile, with assistance from the computer aided diagnosis software based on CAIMEN, the 46 clinicians improved their average top-1 accuracy by 19 & BULL;1% (0 & BULL;345-0 & BULL;411) and top-3 accuracy by 13 & BULL;0% (0 & BULL;545-0 & BULL;616). Interpretation For mediastinal neoplasms, CAIMEN can produce high diagnostic accuracy and assist the diagnosis of human experts, showing its potential for clinical practice. Rhenium diselenide (ReSe2) has attracted great interest due to its unique anisotropic structure and unusual in-plane anisotropic electrical and optical properties. However, efficient fabrication of large-area and high-quality 2D ReSe2 continuous films has become an increasingly important challenge. In this work, centimeter-scale 2D ReSe2 continuous films with the layer number varying from monolayer to 12 layers were successfully grown on a mica substrate using our space-confined CVD system via changing the position of the substrate. The fluorescence quenching effect and surface enhanced Raman scattering (SERS) effect on 2D ReSe2 films with different layer numbers were investigated using rhodamine 6G (R6G) dye molecules as a Raman probe. The large-area ReSe2 films show the layer-number-dependent nature of the SERS effect and a robust suppression effect of fluorescence. Our work explores the practical application of 2D ReSe2 films for molecular detection via the SERS technique.

The reduction of soluble U(VI) to insoluble U(IV) by photocatalytic technology is considered to be a valid method to remove U(VI) from aqueous. Herein, g-C3N4/Ag/TiO2 Z-scheme heterojunction was synthesized for photocatalytic U(VI) reduction application. The SEM, XRD and XPS characterization results showed that the ternary g-C3N4/Ag/TiO2 composite photocatalyst was synthesized successfully. g-C3N4/Ag/TiO2 exhibited excellent photocatalytic reduction performance for U(VI) under visible light irradiation. After 30 min irradiation, the removal rate of U(VI) was above 99%. XPS indicated that the majority of U(VI) on the surface of g-C3N4/Ag/TiO2 was reduced to U(IV). In addition, the photocatalytic activity of g-C3N4/Ag/TiO2 has been kept significantly after five rounds of experiments, indicating the good stability. g-C3N4/Ag/TiO2 exhibited better photocatalytic reduction of U(VI) under visible light irradiation, which is mainly ascribed to Z-scheme photocatalytic mechanism assisted by the LSPR effect (Local Surface Plasmon Resonance). Ag with plasmon resonance effect on the loading has a strong absorption of photon energy. In addition, an intermediate charge transfer channel is formed between Ag and the semiconductor to inhibit the combination of photogenerated electrons and holes, resulting in a significant increase in the photocatalytic activity of the photocatalyst. This idea has some significance in design of other composite photocatalytic system.

Diabetes foot ulcer (DFU) is a serious complication of diabetes, characterized by impaired vascular function, limited angiogenesis, and chronic inflammation. Direct stem cell injection on treating DFU is far from satisfactory in clinical practice, as this therapy neither protects nor localizes the injected cell suspension at the chronic ulcer site. Meanwhile, most of injected cells gradually perished within several days due to senescence or apoptosis. Acellular dermal matrix (ADM) has the potential to act as excellent cell delivery vehicles, considering it is highly biomimetic to native dermal tissue, has low immunogenicity, and suitable for stem cell attachment and proliferation. Hypoxia culture has significantly enhanced effects on the survival ability of in vitro cultured stem cells, indicating this culture mode is a suitable way for inhibiting the senescence or apoptosis of transplanted cells. In the current study, we, respectively, culture adipose-derived stem cells (ADSCs) on an ADM membrane under a hypoxia or normoxia condition to construct two kinds of tissue-engineered dressing membranes (H-ADSCs/ADM and N-ADSCs/ADM) and then comparatively evaluated their efficacy on DFU healing using a diabetic rat model. In vitro results showed that hypoxia precondition could stimulate the ADSCs secreting VEGF-A, and the culture medium from hypoxia-preconditioned ADSCs could enhance the proliferation, migration, and angiogenesis of HUVECs. In vivo results indicated that compared to the N-ADSCs/ADM membrane, the transplanted cells in the H-ADSCs/ADM membrane can survive longer at the chronic ulcer site, consequently improve angiogenesis, inhibit inflammation, and increase extracellular matrix remodeling, eventually accelerating DFU closure. This study provides an innovative covering graft for the treatment of DFU in the clinic.

Extraction of uranium from low-permeability sandstone is a long-standing challenge in mining. The improvement of sandstone permeability has therefore become a key research focus to improve the uranium leaching effect. To address the low-permeability problem and corresponding leaching limits, leaching experiments are performed using newly developed equipment that could apply low-frequency vibration to the sandstone samples. The test results indicate that low-frequency vibration significantly improves the uranium leaching performance and permeability of the sandstone samples. The leaching effect of low-frequency vibration treatment is approximately nine times more effective than ultrasonic vibration treatment, whereas the concentration of uranium ions generated without vibration treatment is not detectable. Mathematical model that considers the combined action of physico-mechanical vibration and chemical erosion is established to describe the effect of low-frequency vibration on the permeability. The calculated results are in good agreement with the tested permeability values. This study thus offers a new method to effectively leach more uranium from low-permeability sandstone using CO2+O-2 and provides an insight into the impact of low-frequency vibration on the uranium leaching process. Background: The prevalence of stroke in young adults is increasing. We investigated the monogenic basis of young adult cryptogenic stroke patients. Methods: This multicenter study enrolled cryptogenic stroke patients under 55 years old, and individuals with nonstroke diseases were included as controls. Targeted next-generation sequencing (NGS) was applied with a custom-designed gene panel that included 551 genes. Rare variants were classified into 2 groups: pathogenic variants and variants of unknown significance. Results: A total of 153 individuals, including 30 (21 males, 70%; mean age 36.1 +/- 10.2 years) in the disease group and 123 (59 males, 48.0%; mean age 40.4 +/- 13.1 years) in the control group, were recruited. In the disease group, 32 rare variants were identified. Among these individuals, 18 pathogenic variants in 16 patients were detected, with a 53.3% (16/30) diagnostic yield of monogenic causes for cryptogenic stroke. None of these mutations were observed in the control group. Among the mutant genes, the most prevalent were Notch receptor 3 (NOTCH3), protein kinase AMP-activated noncatalytic subunit gamma 2 (PRKAG2), and ryanodine receptor 2 (RYR2). Genes associated with cardiogenic diseases showed the highest mutation frequency (10/18, 55.6%) followed by genes associated with small-vessel diseases (SVI)s) and coagulation disorders. None of the patients with mutations had evident abnormalities in the heart or other systems checked by routine tests. For the imaging phenotype-genotype association analysis, infarctions in both the anterior and posterior cerebral circulation were only observed in patients with genes related to cardiogenic disease. Conclusions: In this study, pathogenic variants were identified in nearly half of the young-onset cryptogenic stroke patients, with genes related to cardiogenic diseases being the most frequently mutated. This may have implications for future clinical decision-making, including the development of finer and more sensitive examinations.

This retrospective study investigated the association between the pattern of disease progression and molecular mechanism of acquired resistance in a large cohort of 49 patients with ROS1-rearranged advanced non-small-cell lung cancer treated with first-line crizotinib. We found that treatment-emergent ROS1 point mutations were the major molecular mechanism of crizotinib resistance, particularly for patients who developed extracranial-only disease progression. Our findings highlight the importance of rebiopsy and gene testing for subsequent-line therapeutic management.

Objective: To explore the neural network mechanism of Parkinson's disease (PD) with different degrees of depression using independent component analysis (ICA) of the functional connectivity changes in the forehead, limbic system, and basal ganglia regions. Methods: 106 PD patients were divided into three groups: PD with moderate-severe depression (PDMSD, n=42), PD with mild depression (PDMD, n=29), and PD without depression (PDND, n=35). Fifty gender- and age-matched health objects were recruited as a control group (HC). Three-dimensional T1-weighted image and resting-state functional magnetic resonance imaging (RS-fMRI) data were collected. Results: Different functional connectivity was observed in the left precentral gyrus, right precuneus, right inferior frontal gyrus, right medial and paracingulate gyrus, left supplementary motor area, right brain insula, and the inferior frontal gyrus of the left orbit among 4 groups (ANOVA, P<0.05, Voxel size>5). Both PDMD and PDMSD exhibited an increased functional connectivity in the superior-posterior default-mode network (spDMN) and left frontoparietal network (LFPN); they also exhibited a decreased functional connectivity in the interior Salience Network (inSN) when compared with the PDND group. The functional connectivity within the inSN network was decreased in the PDMSD group when compared with the PDMD group (Alphasim correction, P<0.05, voxel zise>5). Conclusion: PD with different degrees of depression has abnormal functional connectivity in multiple networks, which is an important neurobiological basis for the occurrence and development of depression in PD. The degree of decreased functional connectivity in the inSN network is related to the degree of depression in PD-D patients, which can be an imaging marker for PD to judge the severity of depression.

This study intends to explore miR-34a's effect on BMSCs osteogenic differentiation of hyperlipidemia. 20 SD rats were equally assigned into control group (normal diet) and high-fat group (research diet). Cells were transfected with miR-34a mimic or negative control followed by meausring miR-34a, Dvl2, PPAR-gamma, ALP, Runx2, and sp7, and ALP activity. The number of mineralized nodules, adipocytes, miR-34a and PPAR-gamma expression in high-fat group were significantly increased and Dvl2, ALP, Runx2, and sp7 mRNA showed opposite profiles. Meanwhile, Runx2, ALP protein, cytoplasm and nuclear blue-black particles, Dvl2 protein and mRNA in miR-34a mimic group were significantly downregulated (P < 0.05). Additionally, the luciferase activity of wild-type plasmid+ miR-34a mimic group was significantly lower than mutant group, indicating that miR-34a targets Dvl2. In conclusion, miR-34a inhibits the osteogenic differentiation of hyperlipidemia BMSCs by inhibiting the expression of Dvl2, Runx2 and ALP activity, indicating that it might be target in the hyperlipidemia treatment.
