摘要:
严重急性呼吸系统综合征冠状病毒2 (SARS-CoV-2)可通过血管紧张素转换酶2(ACE2)、细胞外基质金属蛋白酶诱导子(EMMPRIN/CD147)、葡萄糖调节蛋白78(GRP78)途径感染宿主细胞,导致新型冠状病毒病(COVID-19),同时引起急性心肌损伤和心血管系统的慢性损伤.因此,在治疗新型冠状病毒病过程中应特别注意心脏的保护.本期继续特邀相关领域专家对新型冠状病毒感染及心肌损伤进行介绍,旨在加深对这方面的认识,提高防治水平.

摘要:
Background Aortic dissection(AD) is a life-threatening vascular disease caused by various etiologies including Marfan’s syndrome. Angiotensin receptor blocker(ARB) drugs have been confirmed beneficial for patients with Marfanoid AD but remain unclear for non-Marfanoid case. Methods Patients with DeBakey type Ⅲ,Standford A aortic dissection were consecutively recruited to Guangdong Provincial People’s Hospital and Jieyang People’s Hospital between January 2010 and January 2015, with 25 individuals in the ARB group and 32 individuals in the non-ARB group(control group), respectively. The follow-up was performed by database or telephone. The follow-up data consisted of patient’s symptoms and chief complaints, blood pressure, heart rate, and aortic computed tomography angiography(CTA) data. Endpoints and in-hospital events were recorded. Results Baseline data, including age, gender, heart rate, smoking status, and the incidence of diabetes, demonstrated no significant difference between two groups. There was neither death nor urgent surgical case in either group during the follow-up period. The aortic CTA follow-up demonstrated the absorption of the proximal inversely avulsed hematoma for both groups, whereas the reduction was more evident in the ARB group as compared with the non-ARB group [-(9.33±1.99) mm vs.-(4.08±2.35) mm, respectively, P<0.05], suggesting that hematoma absorption was more remarkable in the ARB group. The maximum vascular diameter at the dissection in both groups declined during the follow-up, whereas the reduction in the ARB group was significantly more striking relative to that in the non-ARB group [-(5.50±1.15) mm vs.-(0.31±0.69)mm, respectively, P<0.001], indicating a potential role of ARB drugs in curbing aortic enlargement and remodeling. Conclusions Independent of blood pressure(BP) reduction, ARB utilization in patients with non-Marfan’s aortic dissection may facilitate the absorption of reverse-torn hematoma at the proximal end of dissection, as well as delaying the aortic dilation and remodeling.[S Chin J Cardiol 2020;21(3):163-167]

摘要:
The therapeutic effect of metformin (Met) on atherosclerosis was studied here. Effects of methionine and Met on the induction of inflammatory response and H2S expression in peritoneal macrophages were evaluated. Enzyme-linked immunosorbent assay, immunohistochemistry assay, western blot, and quantitative reverse transcription polymerase chain reaction were conducted to observe the levels of cystathionine gamma-lyase (CSE), DNA methyltransferases 1 (DNMT1), DNMT3a, DNMT3b, tumor necrosis factor (TNF- alpha), interleukin 1b (IL-1 beta), and hydrogen sulfide (H2S). Luciferase and bisulfite sequencing assays were also utilized to evaluate the CSE promoter activity as well as the methylation status of CSE in transfected cells. Methionine significantly elevated Hcy, TNF-a, H2S, and IL-1 beta expression while decreasing the level of CSE in C57BL/6 mice. In contrary, co-treatment with Methionine and Met reduced the detrimental effect of Methionine. Homocysteine (Hcy) decreased H2S expression while promoting the synthesis of IL-1 beta and TNF-alpha in THP-1 and raw264.7 cells. Treatment of THP-1 and raw264.7 cells with methionine and Met reduced the activity of methionine in dose dependently. Moreover, Hcy increased the expression of DNMT and elevated the level of methylation in the CSE promoter, whereas the co-treatment with methionine and Met attenuated the effects of Hcy. Methionine significantly decreased plasma level of CSE while increasing the severity of inflammatory responses and plasma level of Hcy, which in turn suppressed H2S synthesis and enhanced DNA hypermethylation of CSE promoter to promote the pathogenesis of atherosclerosis. In contrary, co-treatment with methionine and Met reduced the detrimental effect of methionine.

胞红蛋白(cytoglobin,CYGB)是一种具有抗氧化作用的珠蛋白.本文旨在探讨人脐静脉内皮细胞(Human Umbilical Vein Endothelial Cells,HUVECs)在氧化损伤过程中CYGB的表达;研究CYGB在氧化应激过程中对HUVECs的保护作用机制.结果表明GYGB在oxLDL引起的内皮细胞损伤中能发挥拮抗作用,可能与PKG信号通路有关。

摘要:
为了探讨自噬在FGF21对抗心肌细胞缺氧复氧损伤中的作用.将H9C2心肌细胞分成三组:对照组;缺氧复氧组;缺氧复氧+FGF21组.MTT测定心肌细胞存活率,台盼蓝染色观察细胞死亡情况,试剂盒检测培养液中CK、CK-MB、cTnT、cTnI、LDH、MDA、SOD及GSH-Px含量,Western blot分析LC3Ⅱ/Ⅰ、Beclin1、p62蛋白表达,Ad-mCherry-GFP-LC3感染后细胞自噬流的检测.结果表明FGF21可以减轻心肌细胞缺氧复氧损伤,与增强自噬流有关。

摘要:
目的 研究2019冠状病毒病(COVID-19)轻症患者的临床特点和核酸转阴时间.方法 回顾性纳入2020年2月12日至2月29日武汉黄陂方舱医院的2019新型冠状病毒(2019-nCoV)核酸阳性的患者,分为轻型和普通型,随访至2020年3月10日,分析患者的临床特点和核酸检测结果.结果 本研究共纳入75例患者,平均年龄41.2±11.5岁,男性占52.0%.患者中有发热表现者44.0%,CT影像阳性者54.7%.首次核酸检出阳性率69.3%,核酸转阴天数中位数为9.0天(6.0~14.0天).随访至2020年3月10日,无患者转为重型或死亡.轻型(34例)和普通型(41例)患者的年龄、白细胞计数差异无显著性.与普通型组比较,轻型组女性占61.8%,显著高于男性(P<0.05);两组患者核酸转阴天数差异无显著性[8.5天(7.0~13.8天)比9.0天(5.0~14.0天),P=0.973].亚组分析发现轻型患者中,女性的核酸转阴时间少于男性(P=0.020),服用连花清瘟胶囊亚组核酸转阴时间缩短(P=0.026).结论 相同治疗下轻型和普通型患者核酸转阴时间差异无显著性,轻型患者中女性和服用连花清瘟亚组核酸转阴时间缩短;根据临床表现和影像学诊断COVID-19可能漏诊部分轻症患者,急需准确及时的核酸检测或其它病原学、血清学检测方法.

2019新型冠状病毒(2019-nCoV)以血管紧张素转换酶2(ACE2)为受体侵入细胞引起机体感染,所引起的"细胞因子风暴"是2019冠状病毒病(COVID-19)爆发和死亡的重要原因。临床资料显示,2019-nCoV除了累及呼吸系统外,还伴随着心脏损伤,如急性心肌损伤、心律失常和爆发性心肌炎等。文章主要阐述2019-nCoV对心脏损伤的主要表现和发病机制及其相关防治策略。

目的:动脉粥样硬化(atherosclerosis,As)进程中血管细胞自噬的改变及低剪切应力对血管内皮细胞自噬的影响.方法:高脂饮食喂饲ApoE -/-小鼠,分别于0、4、8和12周处死.HE和油红O染色法观察As 病变;免疫组织化学法和Western blot检测Beclin 1、LC3和p62蛋白表达;将HUVECs 置于体外灌流系统,分别以低剪切应力(5 dyn/cm2)和正常剪切应力(15 dyn/cm2)处理1 h,Western blot检测观察剪切应力对血管内皮细胞自噬及自噬流的影响.结果:随着As 病变进程的发展Beclin 1、LC3和p62含量显著增加.与正常剪切应力组相比,低剪切应力组Beclin 1和LC3-Ⅱ/LC3-Ⅰ的蛋白表达降低;p62表达增加;巴伐洛霉素孵育2h后,低剪切应力组LC3-Ⅱ/LC3-Ⅰ无明显变化,而正常剪切应力组LC3-Ⅱ/LC3-Ⅰ明显增大.结论: As进程中血管细胞自噬障碍,低剪切应力抑制血管内皮细胞自噬.

摘要:
We conducted a meta-analysis to evaluate the effect of probiotic combined with aminosalicylic on induction remission maintenance treatment of ulcerative colitis (UC). We conducted systematic searches in several Chinese and English databases from inception to June 2018, screening randomized controlled trials about effect of probiotics combined with aminosalicylic acid on UC. The evaluation indicator was the rate of remission. The relative risk (RR) and 95% confidence interval (CI) were calculated. A total of 27 studies with 1942 patients were included. The results indicated that the remission rate was significantly higher in the group using probiotics combined with aminosalicylic acid than that in the group using aminosalicylic acid alone (RR = 1.40, 95% CI: 1.27–1.53, P=0.000). The subgroup analysis indicated that probiotics combined with aminosalicylic acid can significantly elevate the remission rate in both mild to moderate (RR = 1.33, 95% CI: 1.16–1.54, P=0.000) and active stage (RR = 1.40, 95% CI: 1.27–1.64, P=0.000) UC. In different number of bacterium, drug types and treatment periods, the combination with probiotics can significantly increase the remission rate UC. The funnel plot shows slight publication bias. Probiotics in conjunction with aminosalicylic can obviously increase the clinical remission rate of activity UC than drug alone. There was no significant difference between combined with mesalazine group and salicylazosulfapyridine group. c 2019 The Author(s).

摘要:
动脉粥样硬化(As)是一种好发于大、中等动脉的由多因素导致的复杂的血管壁慢性病变.其病理特征包括脂质蓄积、平滑肌细胞迁移入内膜并增殖、炎性细胞浸润、胶原沉积、斑块形成等.越来越多的证据表明,NLRP3炎症小体、细胞焦亡、外泌体、肠道菌群、外周血管脂肪组织、非编码RNA等与As的发生发展紧密联系在一起,并成为目前的研究热点.对上述因素在As发生发展中的作用的深入研究将有助于进一步理解As的发生机制及寻找新的有效治疗途径和生物标记物.文章对近两年来中国学者围绕上述方面进行的As基础研究进展作一综述.

Purpose: The goal of present study is to document our single-center experience with chimney technique for aortic arch diseases. Patients and methods: From August 2012 to October 2017, 226 patients (mean age 54 ±12 years; 197 men) with aortic arch diseases underwent thoracic endovascular aortic repair combined with chimney stents. The aortic stent-grafts were deployed in zone 0 (n=22), zone 1 (n=13), or zone 2 (n=191). Results: The technical success rate was 84% (189/226) and immediate type Ia endoleak (ELIa) happened in 37 (16%) patients. The 30-day mortality and morbidity rates were 2% (4/ 226) and 4% (8/226), respectively. Major adverse events include four major strokes, three spinal cord ischemia and one aortic rupture in the early-term. The clinical and imaging follow-up rates were 98% (218/222) and 78% (173/222), respectively. The average lengths of clinical and imaging follow-up were 22±16 months and 20±15 months, respectively. Chimney stent obstructions in left subclavian arteries were recorded in six (3%) patients. During follow-up, five patients died (2%) and two major strokes occurred (1%). One patient (0.5%) underwent reintervention. Conclusion: The current study documented that the chimney technique is effective and safe for treating aortic arch diseases in different aortic zones. Cautions are needed to assess the permanency of chimney stent and to reduce the immediate ELIa rate. © 2019 Huang et al.

摘要:
Purpose: Eukaryotic initiation factor 2α (eIF2α) plays important roles in the proliferation and survival of pulmonary artery smooth muscle cells (PASMCs) in animal hypoxia-induced pulmonary hypertension models. However, the underlying mechanism remains unknown at large. Autophagy has been reported to play a key role in the vascular remodeling in pulmonary arterial hypertension (PAH). The purposes of this study are to determine the functions of eIF2α and autophagy in the vascular remodeling of the monocrotaline-induced PAH rats and to clarify the correlation between eIF2α and autophagy. Methods: We established a rat model of monocrotaline-induced PAH, and we established a cell model of platelet derived growth factor (PDGF)-induced PASMCs proliferation. The vascular morphology and the expression of eIF2α, LC3B, and p62 were assessed in the pulmonary arterial tissue of Sprague-Dawleyrats and PDGF-induced PASMCs. Results: Autophagy was significantly active in monocrotaline model group (MCT)-induced PAH rats, which obviously promotes vascular remodeling in MCT-induced PAH rats. Furthermore, the proliferation of PASMCs was induced by PDGF in vitro. The expression of LC3B, eIF2α was increased in the PDGF-induced PASMCs proliferation, and the expression of p62 was reduced in the PDGF-induced PASMCs proliferation. Moreover, eIF2α siRNA downregulated the expression of eIF2α and LC3B, and upregulated the expression of p62 in PDGF-induced PASMCs proliferation. eIF2α siRNA inhibited the PDGF-induced PASMCs proliferation. Finally, chloroquine can upregulate the protein expression of LC3B and p62, it also can inhibit proliferation in PDGF-induced PASMCs. Conclusion: Based on these observations, we conclude that eIF2α promotes the proliferation of PASMCs and vascular remodeling in monocrotaline-induced PAH rats through accelerating autophagy pathway. © 2019 Guo et al.

摘要:
目的 探讨成纤维细胞生长因子2(FGF-2)对大鼠缺血再灌注损伤心脏保护作用的新机制.方法 在Langendorff装置上建立大鼠离体心脏缺血再灌注(L/R)模型,40只SD大鼠随机分为6组,(1)缺血再灌注组(I/R组)(n=7):K-H液灌流平衡30min,接着K-H液再灌流5 min,停灌30 min,复灌60 min;(2)FGF-2组(n=7):在缺血再灌注组的基础上,复灌最初10 min给予含有FGF-2(10 μg/10 mL K-H液)K-H液灌流;(3)FGF-2+ LY294002组(n =7):FGF-2与LY294002分别按FGF-2组和LY294002组的方式与剂量给与;(4) FGF-2+ SH-5组(n =7):FGF-2与SH-5分别按FGF-2组和SH-5组的方式与剂量给与;(5) LY294002组(n=6):在缺血再灌注组的基础上,在停灌前5 min内和复灌的全程使用含有15 μmol/L PI3K特异性抑制剂LY294002 K-H液灌流;(6)SH-5组(n=6):在缺血再灌注组的基础上,给予含有10 μmol/L PKB/Akt抑制剂SH-5 K-H液灌流,SH-5的使用方式与LY294002相同.用生理多导仪测定左心室峰值收缩压(LVSP)、左心室舒张压(LVDP)和左心室内压最大变化速率(±dp/dtmax);氯化三苯四氮唑(TTC)法测定心肌梗死面积,试剂盒检测冠状动脉流出液中乳酸脱氢酶(LDH)活性,Westem blottmg检测Akt、p-Akt、GSK-3β、p-GSK-3β蛋白水平.结果 FGF-2组LVSP较缺血再灌注组增高25.8%,LVDP增加38.5%,±dp/dtmax绝对值分别增加35.9%和52.0%,心肌梗死面积减少42.3%(P<0.01),冠状动脉流出液中LDH活性降低32 8% (P <0.01),p-Akt/Akt、p-GSK-3β/GSK-3β比值分别增加145%和255%(均P<0.01);FGF-2+ LY294002组LVSP较FGF-2组降低12.6%,LVDP降低17.2%,±dp/dtmax绝对值分别降低13.9%和22.4%,心肌梗死面积增加40.2%(P<0.05),冠状动脉流出液中LDH活性增高25 6% (P <0.05),p-Akt/Akt、p-GSK-3 β/GSK-3β比值分别下降21.2%和39.2%(均P <0.05);FGF-2+ SH-5组LVSP较FGF-2组降低10.1%,LVDP降低13.4%,±dp/dtmax绝对值分别下降9.9%和17.5%,心肌梗死面积增加29 5% (P <0.05),冠状动脉流出液中LDH活性升高20 0% (P <0.05),p-Akt/Akt、p-GSK-3β/GSK-3β比值分别降低23.9%和26 2%(均P<0,05).与缺血再灌注组相比,LY294002组和SH-5组心功能、心肌梗死面积、冠状动脉流出液中LDH活性、p-Akt/Akt、p-GSK-3β/GSK-3β比值变化差异无显著性(均P>0 05).结论 FGF-2可拮抗大鼠心脏缺血再灌注损伤,其作用机制与其激活PI3 K-PKB/Akt-GSK-3β信号途径有关.

摘要:
目的 观察冠心病患者血清中脂肪细胞因子内脂素(visfatin)、脂肪细胞型脂肪酸结合蛋白(adipocyte fatty-acid binding protein,A-FABP)水平的变化,分析其与冠心病临床分型、冠状动脉狭窄程度等相关性,为冠心病的早期诊断和危险分层等提供新依据.方法 共纳入102例于2010年至2012年期间在我院行冠状动脉造影术(coronary angiograpy,CAG)的患者.(1)根据临床症状、CAG结果、心电图及血清学标志物水平,将受试者分为非冠心病组35例,平均年龄(60.70±2.53)岁;冠心病组67例,其中冠心病组又分为急性心肌梗死39例,平均年龄(63.90 ±7.08)岁;稳定型心绞痛组28例,平均年龄(63.40±4.27)岁.(2)依据患者冠状动脉造影检查结果中冠状动脉病变支数的不同,按照美国心脏病协会(AHA)规定,将入选的102例患者分为四组:非冠心病组35例,冠心病单支病变组26例,冠心病双支病变组20例,冠心病三支病变组21例.(3)按照血清Visfatin水平中位数(M=0.27 μg/L)将患者分为Visfatin< 0.27 μg/L组和Visfatin ≥0.27 μg/L组;根据血清A-FABP水平的中位数(P50=24.79 μg/L)将所有患者分为A-FABP< 24.79 μg/L组和A-FABP≥24.79 μg/L组.冠状动脉病变狭窄程度用冠状动脉病变支数和Gensini积分表示,采用ELISA法检测血清Visfatin、A-FABP水平,分析其与年龄、收缩压、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、超敏肌钙蛋白T(cTnT)、高敏C反应蛋白(hs-CRP)、氨基末端脑利钠肽前体(NT-proBNP)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDLC)、低密度脂蛋白胆固醇(LDLC)、空腹血糖(FPG)水平和Gensini积分的相关性.结果 (1)与非冠心病组比较,冠心病组血清Visfatin、A-FABP水平明显升高,差异具有统计学意义(P<0.05).(2)血清Visfatin、A-FABP水平在非冠心病组、冠状动脉单支病变组、冠状动脉双支病变组及冠状动脉三支病变组依次升高,差异有统计学意义(P<0.05),同时冠心病组血清Visfatin、A-FABP水平随着Gensini积分的增加有逐步上升趋势.(3)双变量相关分析显示,血清Visfatin、A-FABP水平与年龄、收缩压、TG、LDLC、TC、FPG、CK-MB、cTnT、hs-CRP、NT-proBNP、病变血管支数、Gensini积分呈正相关,与HDLC水平呈负相关(P<0.05);多元线性回归分析显示,血清Visfatin、A-FABP水平与CK-MB、cT-nT、hs-CRP、NT-proBNP、Gensini积分存在线性正相关(P<0.05).结论 (1)血清Visfatin、A-FABP水平与冠心病关系密切,尤其与急性心肌梗死关系更为显著,在预测冠心病严重程度中有重要价值,有助于冠心病的危险分层.(2)血清Visfatin、A-FABP水平与冠状动脉狭窄程度密切相关,对评估冠状动脉病变严重程有重要参考价值.(3)血清Visfatin、A-FABP水平与常用冠心病危险分层和心肌损伤生物化学标志物CK-MB、cTnT、hs-CRP、NT-proBNP存在较好的相关性,能反映心肌缺血及心肌损伤的严重程度,联合以上生物化学标志物进行综合评估可提高冠心病患者危险分层的准确性.
