Due to the particularity and complexity of cement solidification, radioactive waste may be released and diffused, causing serious radioactive hazards to equipment, operators and environment. In this study, hazard and operability analysis (HAZOP) and risk matrix were combined to identify and evaluate risks of out-drum mixing cement solidification.
